The cast is designed to evenly distribute weight across the sole of the foot, alleviating pressure on the wound. The cast used for this treatment is typically made of fiberglass or a plastic shell with a bar along the bottom to keep it light in weight. The shell is closely fitted around the leg and foot, ensuring it stays securely in place.

Total contact casting (TCC) is a specially designed cast designed to take weight off of the foot (off-loading) in patients with diabetic foot ulcers (DFUs). Reducing pressure on the wound by taking weight off the foot has proven to be very effective in DFU treatment.
If you keep walking without the cast, the ulcer won’t heal. Wearing a total contact cast helps most ulcers to heal in 6 to 8 weeks. Your healthcare provider will probably change your cast 1 to 2 days after it is put on. After that, you’ll probably get a new cast every week until you are ready to wear shoes.
You will need to attend Mend Wound Clinic every 7 to14 days to have the cast changed. Do not try and remove the cast yourself. You can walk and put weight through your cast, unless you have specifically been told not to by your healthcare provider.
